BATH TOWNSHIP, Ohio — A tragic chain-reaction crash involving three semi-trucks claimed the life of 37-year-old Mazen Mammo of Madison Heights, Michigan, on Wednesday morning, September 10, 2025. The deadly collision occurred on Interstate 75 near State Route 81 in Bath Township, Allen County, and left the highway shut down for hours.
Details of the Crash
According to the Lima Post of the Ohio State Highway Patrol, the accident happened at approximately 10:13 a.m. when Mammo, driving a 2021 Freightliner Cascadia semi, was traveling southbound on I-75.
Authorities reported that Mammo’s truck struck the back of a 2020 Volvo 860 semi driven by 63-year-old Nardini Salvatore of Jacksonville, Florida, who was stopped in traffic. The impact pushed Salvatore’s truck into a 2017 Freightliner Cascadia semi operated by 73-year-old Jean Fervil of McDonough, Georgia, which was also stopped.
The collision caused Mammo’s semi to ignite in flames, with thick smoke billowing over the interstate.
Fatal Outcome
Troopers confirmed that Mammo sustained fatal injuries and was pronounced dead at the scene. His body was transported by Allen Mortuary Services to the Lucas County Morgue for further examination.
Both Salvatore and Fervil were injured in the crash but survived. They were transported by Lima EMS to Lima Memorial Hospital, where they were treated for minor, non-life-threatening injuries.
Extensive Damage to Vehicles
The crash left all three trucks heavily damaged:
- Mammo’s 2021 Freightliner Cascadia — suffered severe fire damage, towed by Beaverdam Fleet.
- Salvatore’s 2020 Volvo 860 — sustained severe damage, towed by Miller Performance.
- Fervil’s 2017 Freightliner Cascadia — sustained moderate trailer damage, towed by Minich Towing.
Highway Closure and Emergency Response
The fiery wreck forced southbound I-75 to close for several hours, creating long backups and disrupting travel through Allen County.
Emergency responders from multiple agencies worked quickly to put out the fire, care for victims, and manage traffic control.
Investigation Underway
The Ohio State Highway Patrol is continuing to investigate the cause of the crash. While heavy traffic congestion was a contributing factor, investigators will determine whether speed, driver distraction, or other issues played a role in the deadly collision.
